Job Description:Home Share Provider The Individualized Funding Resource Centre (IFRC) Society facilitates recruitment for supporting staff on behalf of individuals with disabilities residing in British Columbia. As a Host Agency with Community Living BC (CLBC), we are currently seeking family care providers in Vancouver, North Vancouver and Surrey, to provide care and support in their own home to adults with developmental disabilities on the Home Share Program. The Home Share program is a residential option for adults with developmental disabilities so they can share a home with a contracted Home Share Provider who will provide ongoing support in their own home. Some adults also have physical disabilities and require wheelchair-accessible homes. The Home Share Provider provides positive role models in a safe, structured, home environment where adults can develop social and life skills to ready themselves to reach their full potential for independence. Locations: Current positions available in Vancouver, North Vancouver and Surrey Hours: Hour vary according to the contract and adult being served Accountabilities:Be highly motivated with a proven ability to provide a safe environment to the individuals we serve.Preference for Home Share Providers with Social Services experience and knowledge of non-profit work environments.Be able to provide suitable accommodation (ownership or lease), to be the primary residence of the individual.Reside in the Lower Mainland and Fraser Valley.Cooperate with any plan established by the IFRC, or the CLBC, for the placement of the individual.Establishes safety and emergency policies and procedures to provide support, care and supervision consistent with the Personal Service Plan of the Individual.Ensures that the Individual and their support network of family, friends, Community Support Workers in addition to the IFRC are involved in all planning and decision making on matters affecting the Individual, whenever possible.Provide flexible and ever-evolving support to the adults changing needs within the home.Works within the guidelines and instructions within the Home Sharing Services Handbook and all other policies and standards set for the Home Sharing Program.Has a desire to assist individuals to achieve their optimal independence in the community.Assists the individual in making effective choices concerning the development and maintenance of friendships and relationships.Conducts regular and ongoing communication with the individual’s support teams, maintaining clear and professional relationships.Be comfortable working individually and as part of a multi-disciplinary team of professionals. Qualifications (or be willing to obtain): Have a minimum of one year of experience working with individuals with disabilities. A positive Criminal Record Check.Significant and recent experience providing care to individuals facing multiple barriers.Positive Behaviour Support (PBS) Training an asset.Have a minimum of one year of medical and/or community support experience (i.e. care aide, nursing, community support work, mental health worker, addiction counsellor, social work, etc.)Experience working with an individual with difficult/challenging behaviours.Emergency First Aid (Adult Care).CPR (level A). Additional Qualifications (or be willing to obtain):Food Safe Certificate.A Negative TB test.A Valid BC Driver’s Licence, and a driver’s abstract from ICBC.Multilingual skills and multicultural competence are assets.
